Description of internship (duties, hours, population served): *
AMITA Health is one of only 28 awardees across the country to implement the Accountable Health Communities (AHC) Model. The program involves systematically screening Medicare and Medicaid beneficiaries for Social Determinants of Health (health-related social needs) and providing assistance with connecting the patient with community-based resources to resolve those needs. The program screens for five (5) core needs: Housing, Utilities, Food, Transportation, and Interpersonal Safety. This 5-year, CMS funded program will run until April, 2022. This opportunity puts you on the front lines of discovery in the quest for how we integrate social care into the clinical setting and provide value-based care. You’ll gain experience working directly with patients being seen in our Emergency Room (ER), Inpatient (IP), or primary care setting and working alongside healthcare professionals. (Should COVID-19 restrictions require, back-up plans for remote screening have been developed.) Internship assignments are available at any of our 14 ED locations as well primary care settings throughout the Chicagoland area.
