Complete intakes, assessments, and support plans for self-referred clients seeking counseling, financial assistance, and furniture services. Maintain case records and complete reports in compliance with state and agency mandates. Record all actions taken for each case utilizing the State of Illinois DCFS database and related filing system. Facilitate referrals and applications services to the appropriate providers within the community. Perform follow-up services with participants to determine status of case and provide further assistance if needed. Pick-up and deliver furniture to participants’ homes. Keep organized and updated information about community resources and services. Update program records on a monthly basis. Participate collaboratively in community activities, networking, advocacy, and FAC outreach. Hours will vary on student.
